Dear Student,

Adolescence is a time of great change in a person's life as they move towards full adulthood. This includes profound shifts in their outlook, emotions, and actions.

We are a group of scientists from 5 European countries: Greece, Cyprus, Spain, Poland and Romania who developed this survey as a part of an international project iLearn4Helth specifically for students of your age, in order to get insight of the ways in which adolescents process information, as well as provide a safe space for them to share their thoughts and feelings on a variety of topics that may be troubling to them.

Your answers will help us draw conclusions and create online games tailored to your needs and your level of acquisition of knowledge in the field of health education. The questionnaire, thus, serves as a research for the further development of the innovative games in the topic of health promotion in primary schools that will be available for you for free.

First, we'd like to have your input on certain questions and in particular:

1. specifics about yourself and your loved ones
2. concerns that are unique to the adolescent years
